Consent Forms
The Anatomical Gift Program may accept donations when a valid Donor Consent or Substituted Consent form has been completed.
In addition to consent, the donation must meet the conditions required for education, preservation and use, as determined by the Anatomical Gift Program Team.
If the above conditions are not met or if the program does not have the capacity or resources to accept the donation at the time of passing, the program may need to respectfully decline the donation. In such cases, final arrangements for the deceased remain the responsibility of the family or designated representatives.
