The Donation Process
To make the donation process as simple as possible, we have outlined two paths to registration:
- Self-Enrollment – You are completing a consent form for yourself.
- Enrollment by substituted consent – You are registering someone as a donor after they have passed away because they can no longer give consent. If you are enrolling someone else, you must have the legal authority to do so under the PEI Human Tissue and Donation Act. See the FAQs for more information
Self Enrollment
Those individuals choosing to register as a donor with the program should talk to their loved ones about their decision and choose a contact whom they designate to carry out their wishes after passing.

Steps for Self-enrollment:
- Choose a designated contact person – Pick someone you trust to carry out your final wishes.
- Complete and sign two copies of the Donor Consent Form for Self-Enrollment.
- File one copy of the form with your designated contact who will be able to carry out your wishes at the appropriate time.
- Send the second signed copy of the form to the Anatomical Gift Program. This can be done in two ways:
- Make an in-person appointment to drop off the completed forms. To book an appointment call 902-626-4855 or 902-620-5398, or email anatomicalgift@upei.ca
- Mail the completed forms to: Anatomical Gift Program, Faculty of Medicine, University of Prince Edward Island, 550 University Avenue, Charlottetown, PE C1A 4P3.
- After passing, have your designated contact reach out immediately by calling program staff at 902-388-6101. We will guide them through the next steps.
Enrollment by Substituted Consent
After passing, you may still enroll your loved one in the program if you have the legal authority to do so. The following may provide substituted consent, in this priority order:
- A legal guardian of the person of the deceased before death;
- The spouse of the deceased;
- A child of the deceased;
- A parent of the deceased;
- A brother or sister of the deceased.
To enroll a donor using substituted consent, please complete the Substituted Consent Enrollment form.
